Steps for submitting a public information request

Public information requests (also called Open Records Requests) are required to be submitted in writing. We have created a public information request form听for your convenience. However, you are not required to use that form.

• Please make sure that you include a detailed description about the information you are requesting听听听听听听 听听听
• Send your request to the agency鈥檚 Public Information Officer.
• We may need to contact you to clarify your request to make sure we provide you with the information appropriate to your request.
